Outpatient Therapy

Healthy Families C & S provides outpatient mental health therapy to children, families, and adults to help with a wide variety of emotional and/or behavioral problems.

Outpatient therapy is provided at our main office, Monday - Friday. Therapy is also provided at 3 satellite offices located in Richmond, Excelsior Springs and Platte City.

Healthy Families C & S strives to provide therapy regardless of ability to pay. We offer a sliding scale. Healthy Families C & S also works with a variety of insurance companies, as well as MC+, Medicaid, and DFS’s Children’s Treatment Services (CTS). Referrals come from a variety of sources, including Community Mental Health Centers, Schools, Physicians, Health Departments, Division of Family Services, Juvenile Justice, self-referrals, case managers, etc.

Intensive Community Support

Healthy Families C & S provides Intensive Community Support (ICS) as a contract provider for Tri-County Community Mental Health Services (TCCMHS). This service is an intensive in-home program with an average length of 45 days. ICS is designed as a crisis intervention for children/adolescents with severe emotional and behavioral problems that have placed them at significant risk for out of home placement. Services are provided to the child and their family. The intervention is designed to help stabilize the child in the home and community and link them with follow up services.

Healthy Families C & S ICS workers have Masters Degrees in social service fields such as Counseling and Social Work. They work a flexible schedule to meet the needs of the families they serve, providing 6-10 direct contact hours per week to each family served. Workers are available to clients 24 hours a day for crisis situations.

Targeted Case Management (TCM)

Healthy Families C & S provides Targeted Case Management as a contract provider for Tri-County Community Mental Health Services (TCCMHS). This service is a 3-6+ month program for Severely Emotionally Disturbed children who present with problems involving multiple domains and service providers. Targeted Case Management is an in-home service that provides support and linkage to community resources and monitors service delivery to the child and family. The TCM specializes in working with the Division of Family Services (DFS), Juvenile Offices, Schools, Doctors, Other Treatment Providers, and Community Resources to provide comprehensive treatment planning designed to keep the children in the home and least restrictive environment.

Healthy Families C & S Targeted Case Managers have Masters Degrees in social service fields such as Counseling and Social Work. Healthy Families C & S Targeted Case Managers work a flexible schedule to meet the needs of the families they serve. Workers are available to 24 hours a day for crisis situations.

Healthy Families C & S Targeted Case Managers carry a caseload of 10-12 clients and provide approximately three hours of service per week to each client.

School Resource Counseling Services

School Resource Counseling is a service provided by Healthy Families C & S that provides school districts with a variety of different counseling services to help students who are having social, emotional, behavioral, family or academic problems. All services are tailored to meet each district’s requirements and can include:

Individual counseling: Individual sessions to help provide support to deal with (for example) management of feelings and behaviors, social skills, problem solving/conflict resolution, and family issues.

Group counseling: Small groups that address topics to help students manage their behavior, to increase their self-esteem, social skills, and relationship skills and to help them learn to problem solve and resolve conflicts.

Case management: The School Resource Counselor works closely with school officials, parents, and local social service agencies to help ensure that information and/or referrals for appropriate social services has been provided to a student’s family.

Classroom guidance: Working closely with the school’s own counselor, classroom lessons are developed and presented on topics recommended by the school staff. These topics can include: understanding feelings, hygiene topics, and personal safety issues.

Family meetings: Family contact, including family therapy sessions, are made as needed, at the request of a staff member, or at the request of the student’s guardian. Contacts will provide an update on the student and will gather information from the family with regard to their observations.

Teacher consultations: The Resource Counselor is available to meet with teachers to discuss their choice of behavioral issues and how to better manage these in the classroom.

Student observations: Observations can be done at the request of the guardian, school, or by another mental health professional. The Resource Counselor will observe the student in the student’s own classroom in a variety of classroom activities. Observations will be noted and will be used to help create a treatment plan for the student.

Communication with other mental health professionals: At the request of the guardian, written or verbal reports on the student’s current academic and behavioral progress can be assessed and reported.

Behavior Aide Services

Healthy Families C & S has several contract staff available at any one time to provide behavior aide services to families. These services are primarily contracted by TCCMHS and DMH, but occasionally are contracted directly by the family.

Behavior Aides work with families with Severely Emotionally Disturbed (SED) Children. They work in conjunction with a Targeted Case Manager (TCM) to help implement behavior management plans, teach parenting skills, and provide support.

Internship/Practicum Programs

Healthy Families C & S works with a variety of schools to provide internship and practicum experiences for Masters and Bachelors level students.

Interns are provided with close supervision and exposure to many or all of the programs SOS provides.

Groups

Healthy Families C & S looks to provide groups periodically throughout the year to help meet the Mental Health needs of children and families. It is typical for one group to be running at any one time.

Special Programs

Healthy Families C & S periodically provides special programs throughout the year such as Summer Activity Programs and Family Support Programs for children and families to provide support and education.

Consultations/Community Presentations

Healthy Families C & S staff are available throughout the year to provide consultations and presentations to agencies, groups, and organizations in the communities we serve.

 

 

 

 

 

 

 

 

 

 

 

BraveHearts for Dads

BraveHearts for Dads is a free program that aims to strengthen father/child relationships by promoting positive parenting.  Each father attends a 10-week series that focuses upon the importance of actively parenting and provides a safe environment for voicing any concerns.  Free dinner and day care is available at every meeting.  Gas and Wal-Mart cards are given as an incentive for attending the groups. 

Case management is provided for fathers that may need extra support in finding employment, improving his education, or other resources.  One on one mentoring is also available upon completion of the 10-week series.  Monthly “play-days” are offered for the whole family to enjoy an evening out with all the participants that have gone through the program.  Free outings are also available for fathers, mentors, and children to prevent social isolation.  The fathers in BraveHearts just attended a Royals game and will be going to the I-70 Speedways in July. 

The interaction and communication between the fathers has been incredible!  They have all reached out to give each other support and encouragement (makes my job easier) concerning the barriers fathers encounter.  We have watched the fathers not only bond with each other, but with their children.  I wanted to share a few comments that “our” participants have shared with us:
